I certify that I have discharged all financial liabilities to my previous club(s). I certify that, if applicable, I have International Clearance from an overseas Association to play in England. This includes, but is not limited to Wales, Scotland, Northern Ireland and the Republic of Ireland. I certify that I am at least 16 years of age.
